Synergy Care is hiring a full-time Occupational Therapist for an AL/IL/Memory Care community-Bethesda Gardens in Fort Worth, TX!

We are looking for a team-oriented clinician who is passionate about providing quality patient care and making a difference in the lives of others.

 Job Responsibilities:
 Evaluate patients and develop individualized treatment plans
 Provide ongoing treatment and discharge planning
 Supervise assistants and aides
 Participate in case management
 Deliver high-quality occupational therapy services in accordance with state regulations and Synergy Care guidelines
 Represent Synergy Care’s commitment to excellence in every patient and facility interaction

 Qualifications:
• Graduate of an accredited Occupational Therapy program (or graduating within 6 months)
